The Red Eye That Wasn’t Simple Conjunctivitis

Pooja, 27, had a painful, red left eye for two days. She had been prescribed antibiotic drops by a general practitioner. On slit-lamp exam, I found ciliary injection, photophobia, and anterior chamber cells—classic signs of anterior uveitis.

Further questioning revealed joint pain and lower back stiffness. Labs pointed toward HLA-B27 positive seronegative spondyloarthropathy.

We co-managed with rheumatology, and her flares became manageable with systemic therapy.


The Lesson Learned

Always differentiate between conjunctivitis and intraocular inflammation. The red eye can be a window into rheumatology.
