Rupa, 35, came in for routine IUD removal. “I can’t feel the strings,” she said. I couldn’t feel them either. An ultrasound showed an empty uterus.
A pelvic X-ray revealed the device in the abdominal cavity. It had perforated through the uterine wall—silently. Laparoscopy confirmed it lodged near the bladder, thankfully without damage.
We removed it and reinforced patient education on checking strings.
Missing IUD strings require immediate imaging—don’t assume spontaneous expulsion. Even silent perforations can carry silent risks.
