The “Sprain” That Was a Missed Fracture

Meera, 45, twisted her ankle stepping off a curb. She was seen at a clinic and sent home with a “sprain” diagnosis. Three weeks later, she came to me because she still couldn’t bear weight.

On examination, she had tenderness over the fifth metatarsal base. A repeat X-ray showed a Jones fracture—missed due to early imaging without weight-bearing views. Now displaced, it required internal fixation.

The Lesson Learned

Don’t rush to label injuries as sprains. Incomplete or early imaging can mask critical fractures—reassess if recovery doesn’t follow expectations.
