Mr. Ramesh, 60, came to me with stiffness and pain in his left shoulder for six months. Diagnosed as adhesive capsulitis, he was advised physiotherapy and NSAIDs.
I examined him and found firm axillary nodes. A chest X-ray revealed a lung mass. It was Pancoast tumor—pressing on the brachial plexus and mimicking frozen shoulder.
He was referred to oncology and started on treatment.
The Lesson Learned
When common diagnoses don’t respond, think uncommon. Shoulder stiffness in older adults with systemic signs may be more sinister than frozen shoulder.
